- Recent x-rays on a disc or patient portal, if available
- A completed health history, or arrive 15 minutes early
Patients
First visits are longer on purpose.
New patients are scheduled for a comprehensive exam, necessary imaging, and a conversation about what you want from the next year of care—not a ten-minute cleaning and a goodbye.
Insurance & payment
We are in-network or file as out-of-network with most PPO plans, including Delta Dental, Cigna, Aetna, MetLife, and others. We estimate benefits before treatment. HSA and FSA cards, major cards, and staged self-pay plans are accepted.

If you are in pain
Call the office before submitting a form. Swelling, trauma, a tooth that will not stop hurting, or a restoration that came off can often be seen the same day.
